First Transcatheter Aortic Valve Replacement [TAVR] done in the region Dr Bali conducts first non-surgical valve replacement in region

Chandigarh, 19 September, 2018
In another first to his credit, Dr HK Bali, Director Cardiology, Fortis Hospital, Mohali, has become the first surgeon in north India to conduct a non-surgical valve replacement. A team of doctors led by Dr. Bali performed this exclusive Transcatheter Aortic Valve Replacement [TAVR] surgery which till date hasn’t been carried out even at the PGIMER, Chandigarh, itself.  Dr. Bali also did the stenting of a narrowed vessel of the heart. The procedure lasted 2.5 hours and was completely successful. Patient has made complete recovery and has become mobile from the second day after the procedure.
Addressing a press conference here today Dr. Bali informed, ‘’Percutaneous aortic valve replacement is a new treatment option for patients of critical aortic stenosis, in whom surgical risk is very high as it carries increased morbidity. This is the first case of non-surgical aortic valve replacement in this region and comes as a boon to patients of North India. Elderly patients suffering from this critical disease would benefit greatly by this new technology. Valve replacement is done from the groin and the patient can become completely active next day after the procedure. This is a completely non-surgical technique and the groin site is repaired non-surgically immediately after the procedure’’.
Dr. Bali cited the case of one Promila Sharma (name changed) from Mohali who was having symptoms of severe breathlessness on minimal day to day activities. On investigating, the 75-year-old lady was found to have critical narrowing of one of the left-sided valves of the heart – the aortic valve, along with critical narrowing of artery of the heart.
Mrs Sharma was seen by cardiac surgeons and was found to be very high risk for surgical aortic valve replacement. After detailed discussion, the Heart Team of Fortis Hospital, Mohali, decided to offer non-surgical transcutaneous aortic valve replacement as the best treatment option to her family.
Dr HK Bali led a team of doctors to successfully perform the first transcutaneous aortic valve replacement (TAVR/TAVI) of the region.
“Narrowing of the aortic valve is a life threatening disease and normally requires an open heart surgery with valve replacement. Epidemiological studies have determined that more than one in eight people aged 75 and older have narrowing of the left-sided valves of the heart, which is known as aortic stenosis. If not treated emergently, this may lead to severe disability and sudden death”, he informed.
He said that elderly patients suffering from the disease should be evaluated by their cardiologist to review their suitability for this procedure. Symptomatic patients with severe aortic stenosis [AS] must be referred to a Heart Team for valve replacement evaluation.
The Heart team of Fortis Hospital, Mohali would evaluate such patients and individualized decisions would be taken to replace the aortic value non-surgically. The risk of this procedure is comprehensively less than open heart surgery in elderly individuals.
Dr. Bali also stressed on the need for close collaboration between cardiologists and cardiac surgeons to provide optimal care of critically ill patients.
Mrs Promilla Sharma was present during the press conference and interacted with members of the media and shared her experience of undergoing the procedure.
Meanwhile, Abhijit Singh, Zonal Director, Fortis Hospital, Mohali, said, “We at Fortis are proud to say that we are pioneers in Cardiac Care in the North region. The many firsts that we gave the city are Flat Panel Cath Lab, Intravascular Ultrasound (IVUS), Rotablator System, Chronic Total Occlusion Percutaneous Coronary Intervention (CTO PCI), Fractional Flow Reserve (FFR), Optical Coherence Tomography (OCT), and now Transcatheter aortic valve implantation (TAVI). Elderly patients with valve narrowing can benefit by this new non-surgical technique.’’

Preterm baby with complications treated successfully at Fortis Mohali

Chandigarh, September 12, 2018In a unique case, a pre-term baby was successfully treated at Fortis Hospital, Mohali despite several complications. The baby was born through C-section on 6th November 2017 at 23 weeks of gestation (at 5 and half months of pregnancy) and weighed only 560 grams at birth. This is the first 23-week baby to have been saved in the entire region including Punjab, Himachal Pradesh, Jammu & Kashmir.

Dr Swapna Misra, Additional Director, Obstetrics & Gynaecology, Fortis Hospital, Mohali, while interacting with media said A 36 year old, 22 week pregnant woman came to the hospital in an emergency, with bleeding and leakage. She has no previous live issue. She had diabetes and hypothyroidism. This was a very early pregnancy, but parents were keen to continue despite there being a 70 percent chance of bad outcome. The parents agreed for conservative management. After 20 days, she delivered a baby. The Neonatal ICU team looked after the baby.

Giving the details, Dr. Sunil Kumar Agrawal, Consultant, Neonatology, Fortis Hospital, Mohaliexplained, baby of Shweta was born through C-section on 6th November 2017 at 23 weeks of gestation (at 5 and half months of pregnancy) and weighed only 560 grams at birth. At birth his lungs were very poorly developed for which he had to be put on ventilatory support right soon after delivery and was shifted to the Level 3 NICU.

Dr. Agrawal explained, “For the first 2 months of life everyday used to be a new challenge with its difficulty. At times his lung condition used to deteriorate, at times a duct in the heart (PDA) causing problems, at time difficulty in digesting feeds and at times infection is the troubling issue. But with advanced medical and nursing care, gradually he started improving and we could remove him from ventilator at 105 days of life. Even after that he used to require very small amount of oxygen and we could discharge the baby on 140 days of life. At discharge he weighed 2.9 kg and was receiving oxygen, which the parents continued at home for another one month.”

Even after discharge the journey was not smooth. He has to be readmitted 6-7 times with feeding issues, respiratory illness. But now he is fine weighing around 5 kg and as a fighter he conquered all the battels. That’s why we say he is a true fighter and a miracle baby in true sense, he said.

At 23 weeks of life, most of the organs of a fetus are not properly developed and as the chances of survival of such premature babies are very slim, many parents and even doctors believe that these babies cannot survive. Therefore, many a time these babies are not given aggressive intensive care. But with advancement of medical science, such micro preemies can be offered aggressive care if parents wish to do so. However, such babies need long-term follow up for their growth and neurodevelopmental assessment, as per the doctors.

NEW CANCER TREATMENT, CRYOABLATION : NOW AVAILABLE AT PGIMER First in the country

The incidence of cancer is increasing  globally. In India, as well, the number of patients with cancer is on the rise. The department of Radiodiagnosis at PGIMER, Chandigarh has recently acquired the state-of-the-art machine for doing cryoablation of tumours in patients who are not  suitable to be treated with surgery. This machine is the first of its kind installed anywhere in the country. The department is now fully equipped to treat unresectable cancers with all the ablation modalities like radiofrequency ablation, microwave ablation, irreversible electroporation, high intensity focussed ultrasound and now cryoablation, available under one roof.  'With the installation of this machine, the Institution will now be part of the elite group of hospitals in the entire world including the likes of MD Anderson Cancer Center in United States', said Prof. Khandelwal, Head, Department of Radiodiagnosis, PGIMER.
Cryoablation works on the principle of freeze-thaw-freeze cycle. This results in the destruction of the tumor cells by mechanical and inflammatory mechanism. Needles or probes of various sizes are introduced into the tumours of the kidney, lung, liver or bone under the guidance of CT scan or ultrasound. Cryotherapy is then delivered into the tumor over a period of 30 minutes. The advantage of this procedure over the other ablation techniques is that the technique is relatively pain free as it numbs the nerves supplying the organ due to the extreme low temperature (about -150 degree celsius) reached at the site of therapy. Therefore, it may be done under local anesthesia or with conscious sedation only. Five tumours in different locations like lung, kidney, liver and bone were treated by Dr Naveen Kalra and the team of interventional radiologists including Dr Mandeep Kang, Dr Anupam Lal, Dr Mahesh Prakash, Dr Anindita, Dr Ajay, Dr Ujjwal and Dr Shridhar. Dr Francois H Cornelis, Interventional Radiologist from Sorbonne University, Paris, France helped in the planning of the technique and guided the team as these procedures were being performed for the first time in the department and in the country.
